CERTIFICATES, LICENSES, REGISTRATIONS

Valid Florida Driver's License

Current Florida State License for Physical Therapy, Medicaid and NPI numbers preferred

ESSENTIAL FUNCTIONS

• Assess and treat client/residents need for physical therapy and functional level with different environments.
• Develop individual physical management program for each client as needed.
• Create programming to address gross motor functioning, including range of motion, functional mobility, transfers, etc.
• Provide physical therapy input to the interdisciplinary team for the development of care plans.
• Recommend habilitation plans for clients, in collaboration with other disciplines, to be carried out by rehab aides.
• Recommend and monitor use of adaptive equipment for clients as needed.
• Order appropriate materials and equipment.
• Recommend and monitor the use and fit of lower extremity orthotics.
• Participate in meetings, committees and in services as necessary.
• Assist in incorporating carryover of physical therapy goals in school programs and reevaluate functional levels annually.
• In-service appropriate personnel and families in order to implement PT goals.
• Participate in wheelchair clinic and coordinate services with physiatrist, wheelchair vendors, and applicable funding agencies.
• Prepare necessary paperwork for various programs, insurances, and agencies.

SUPERVISORY RESPONSIBILITIES

Assist in the supervision of PTA, therapy aide and PT field work intern. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, assist in hiring, and training employees; planning, assigning, and directing work; assist in appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports, notes and correspondence. Ability to speak effectively before groups of customers or employees of organization.
